Diamond Home Health Care is a CHAP accredited Medicare certified agency that emphasizes a holistic approach to care and recovery from illness, surgery, or injury in an individual's own home. Our goal is to support families and patients while working together with the physician to maximize the individual's ability to improve health, healing and independence. We implement a complete and coordinated plan of care that includes the nurse assigned to the facility acting as "case manager". The Diamond Home Care team includes:
